This approach may be considered for patients who need to replace all or most teeth in the upper or lower jaw with a fixed, implant-supported bridge. It can be an alternative to removable dentures when suitable.
What Is All-On-4?
All-On-4 refers to a method where a full set of replacement teeth is supported by four dental implants placed in specific positions in the jaw. Instead of one implant for each missing tooth, four implants are placed in locations that make the most of the available bone.
Usually, two implants are placed near the front of the jaw and two near the back at a slight angle. This placement helps avoid sensitive areas such as the sinuses in the upper jaw or nerve canals in the lower jaw, and it can sometimes reduce the need for additional bone grafting procedures.
In many cases, a temporary set of teeth can be fitted soon after the implants are placed, allowing the patient to use them while the jawbone and implants gradually integrate. The final bridge is fitted later once healing and stabilisation are complete.
Who May Be Suitable for All-On-4
All-On-4 treatment may be an option for patients who:
- Have lost many or all teeth in the upper or lower jaw
- Find their dentures loose or difficult to manage
- Have enough bone in the front of the jaw to support implants
- Have healthy gums and controlled oral conditions
- Are able to keep up with regular cleaning and ongoing care
However, some factors can make All-On-4 less suitable, such as:
- Not enough bone in important areas of the jaw (sometimes requiring grafting)
- Ongoing or untreated gum disease
- General health issues that may affect healing
- Smoking, which can reduce implant success
- Certain medications or conditions that affect bone strength
At your consultation, the dentist will check your oral health, bone condition, and medical history before explaining whether All-On-4 may be a suitable option for you.
